Plymouth Local Care Partnership is one of five Local Care Partnerships across Devon and is set within the context of the Devon Integrated Care System.
Integrated Care Systems are not new organisations. They are intended to build and strengthen on existing partnerships and relationships across the health and care sector, to drive change, reduce inequalities and lead to better more joined-up care for the benefit of our population, as set out in the NHS Long Term Plan.
Local Care Partnerships are the means at local level that give us the power to identify and focus our efforts on what matters to local people.
In Plymouth we have been doing this for a long time. In 2014 we started our journey to integration and have forged strong relationships between Plymouth City Council, the NHS, primary care and other local health and care providers like Livewell Southwest, alongside our third sector and voluntary, community organisations
We want people to be involved in their own care and treatment and supported to manage their own health and wellbeing daily.
We want people to have the opportunity to work, maintain relationships and friendships, and continue to do the things that make them happy.
Everyone must have a voice, be heard, and understood.
